Reports To: Head of Allied Health Professionals


Job Purpose

To provide emotional and psychological support to patients with life-limiting illnesses, their families, and carers. This role focuses on helping individuals cope with grief, loss, adjustment to illness, end-of-life issues and bereavement in a compassionate, respectful, and professional manner.


Key Responsibilities

Counselling & Support

  • Provide one-to-one counselling to patients, families, and carers.
  • Offer Counselling and Bereavement support.
  • Facilitate support groups where appropriate.
  • Use a variety of therapeutic approaches tailored to individual needs.

Collaboration & Communication

  • Work with the multidisciplinary team (MDT) to ensure holistic care.
  • Participate in staff debriefings and attend meetings.
  • Liaise with external mental health and social care services where needed.

Record Keeping & Confidentiality

  • Maintain accurate and confidential records in accordance with GDPR and hospice policies.
  • Complete clinical notes promptly on Systmone.

Professional Development

  • Engage in regular clinical supervision.
  • Participate in training, workshops, and continuing professional development (CPD).
  • Stay up to date with current best practices in counselling and palliative care.

Essential

  • Accredited counsellor (NCPS, BACP or equivalent).
  • Experience working with grief, loss, trauma, or serious illness.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • High degree of emotional resilience and self-awareness.

Desirable

  • Previous experience in a hospice, palliative, or healthcare setting.
  • Training in bereavement counselling or end-of-life care.
  • Knowledge of safeguarding and ethical practice in a clinical setting.
